Spur maintains workers’ compensation insurance coverage at no cost to our employees. An employee who suffers an on-the-job injury or illness may be entitled to benefits under the applicable state Workers’ Compensation law.
Additionally, Spur offers a limited benefits plan to our employees through Essential StaffCARE. Among the benefits offered in this plan are Medical, Dental, Vision, Short-Term Disability and Term Life Benefits. Employees are able to enroll in the Essential StaffCARE program within 30 days of their hire date or Spur’s annual 30-day open enrollment period in October. If you do not enroll during one of those time periods, you will have to wait until the next annual open enrollment, unless you have a qualifying life event. You have 30 days from the qualifying life event to enroll. You will receive your benefits plan enrollment form with your new hire paperwork.
If you wish to enroll, you must complete the benefits enrollment form (attached below) and fax it to Essential StaffCARE at 1-803-264-0772. Forms sent to Spur will not be able to be processed.
You must complete and return the enrollment form within 30 days of your first day worked as a Spur employee if you wish to enroll. Please do not submit your enrollment form before your first day worked.
If you do not wish to enroll, you will still need to complete the top portion of the enrollment form and then select “No to all Benefits” in the benefits section of the application.
Coverage will begin the Monday following a payroll deduction and continues as long as you have a deduction from your paycheck. Please review your pay stub for deductions. If you miss a payroll deduction, to avoid a break in coverage, you may make a direct payment to PAI. After six consecutive weeks without a payroll deduction or direct premium payment, coverage will be terminated, and COBRA information will be sent at that time.
If 3 weeks have passed and you do not see any deductions, contact ESC directly to inquire about your application/enrollment form at 1-866-798-0803. ESC Customer Service Call Center hours are Monday - Friday 8:30 a.m. to 8:00 p.m. EST. Spanish-speaking representatives are available.
When completing your ESC form, enter 003 and your date of hire at the top of your ESC form for the location. If this information is NOT completed, the form is not legible, or it is not signed, this could delay your start date of Essential StaffCare by several weeks.
